Checkout fields Manager plug-in enables Magento webstore owners to get extra information on checkout page. You can add unlimited checkout attributes like text field, date, yes/no, multiple select. The admin and customer both can view these order attributes. You can also add these fields in invoice email and PDF through configuration options.
